If your child has never attended school before, you need to register your child for *preschool or kindergarten (depending on age). The following forms are required in order for your child to attend school:
An official birth certificate - this must be from the state of birth; hospital certificates do NOT meet this requirement; applications are in the office if needed
A current immunization certificate on a Kentucky form - must be signed by a doctor or practitioner and have the expiration date on it
A physical examination form signed by a physician or practitioner - must be on KY form (see above)
An eye examination form signed by a licensed optometrist or ophthalmologist - this is due by January 1, 2023 - for all three-, four-, five- and six-year-olds - must be on KY form (see above)
A dental examination form signed by a dentist, dental hygienist, physician, registered nurse, advanced registered nurse practitioner, or physician assistant - this is due by January 1, 2023 - for all five- and six-year-olds (not needed for preschoolers) - must be on KY form (see above)

During registration, you will be asked to sign a records release so that we may retrieve a copy of your child's permanent educational record.
